SUMMARY OF SCHOOL OPENING
Pershing County Schools will convene Monday, August 27, 2012. Middle and high school students will attend school from 7:45 a.m. to 3:15 p.m. Elementary students in grades 1 thru 5 will attend from 7:43 a.m. to 3:10 p.m. Kindergarten sessions are A.M. 7:43 to 11:00 a.m. and P.M. from 11:55 a.m. to 3:10 p.m.

IMMUNIZATION REQUIREMENTS - All students in Pershing County, Early Childhood through 12th grade, must present information regarding immunization from certain diseases or provide reasons for exemption. It is recommended that parents contact Mrs. Debbie Pontius, school nurse, at 273-1147. Information about immunizations can also be found on the district website at www.pershing.k12.nv.us. There is a new immunization requirement. All students entering 7th grade MUST have a Tdap (whooping cough, tetanus & diphtheria) immunization. Also, all children in elementary and middle schools who have not had EITHER the chicken pox OR TWO chicken pox immunizations MUST have a SECOND immunization before the beginning of the new school year.
All immunization data supplied to the District will be entered in WebIZ, the NV statewide immunization database unless otherwise directed in writing by parent or guardian of student.
STUDENT HEALTH SCREENING REQUIREMENTS – As mandated by AB354, the school nurse is required to report the results of ALL screenings to the state including student height and weight, vision, hearing and scoliosis information. Parents may decide to opt their student out of the screening by providing written notification to the school nurse. Parents will receive written advance notice of scoliosis screenings. NUTRITION & WELLNESS POLICY – The school district has implemented a Nutrition and Wellness Policy in compliance with federal and state mandated regulations. The purpose of the Nutrition and Wellness Policy is to help educate our students in participating in a healthy eating and living environment. The policy provides the guidelines for acceptable food and/or drink items allowed during school hours. All food/drink items provided by parents for birthday parties and any other special occasions or food/drink items associated with a cold/sack lunch from home are not required to follow the nutrition and wellness policy. The district’s Nutrition and Wellness Policy and approved snack list may be viewed on the district’s website www.pershing.k12.nv.us by clicking on the SCHOOL NUTRION link. SCHOOL BREAKFAST/LUNCH PROGRAM - The School Breakfast and Lunch Programs will begin on the first day of school, Monday, August 27. Meal prices for school breakfasts are $1.50 for elementary students and $1.75 for middle and high school students and adults; lunches are $2.25 for elementary students, $2.75 for middle & high school students, $4.00 for adults and .50 for extra milk. Breakfast will be served at 7:00 to 7:40 a.m. at elementary and middle school. Lunch will be served during each school’s lunch period.
Your family may be eligible for free and reduced meals. Please complete an application. Applications are available at every school or on the District’s website www.pershing.k12.nv.us.
Households receiving assistance under the Food Stamp Program, TANF, SNAP or FDPIR for their children will be notified of their eligibility and that their children will be provided free benefits unless the household notifies the school that it chooses to decline benefits. If your household does not receive a notification regarding your eligibility by September 15, 2012 you need to submit an application to your child’s school.

ACTIVITY FEES
Book and activity fees for school students are as follows:
High school student body cards $30.00 and student book deposits $10.00. Those students who do not have a student body card will be charged $4.00 to attend extra-curricular high school events.
Middle school student book deposits are $10.00, student body fees $10.00, lab fees $5.00, and locker deposit fees $10.00 for a total of $35.00. Middle School yearbooks may be purchased separately. Middle school students who have not paid the Student Body fee will be charged $2.00 for middle school activities.
Elementary school Yearbooks may be purchased at a later date for $15.00.
Admission to extra-curricular high school events will be adults $5.00, middle school and elementary school students $3.00, and senior citizens $3.00. Season passes are available for individuals. Contact the high school office for more information.
WHERE TO MEET THE FIRST DAY – At 7:45 a.m. the first day of school, Monday, August 27th, all high school students will meet in the high school auditorium.
STUDENT INSURANCE AVAILABLE - Students who wish to purchase insurance are encouraged to pay the premium by registration day. Enrollment is available online at: www.k12StudentInsurance.com

STUDENT ACCIDENT INSURANCE PLAN
Every Pershing County School District student will be given the opportunity to enroll in an Accident Insurance Protection Plan.
The Plan covers accidents while a student is at school during activities on or away from school grounds. Additional coverage for students includes all school sports, on or away from school grounds, except interscholastic tackle football; and travel directly to and from home for regular classes, for one hour before and after school, and in school provided vehicles any time. Policy does not cover travel in two or three-wheeled vehicles any time.
Parents are encouraged to read the coverage provided by the insurance policies so that they will be aware of the limitations of the plans.
Premiums for the At-School Accident Plan costs range from $9 to $32 per school year for Early Childhood through 12th Grade students.
Another plan is offered which provides the same coverage as At School Coverage Plan, but the coverage is available 24-Hours a day for an entire year. Premiums range from $25 to $195 depending upon coverage for all grades Early Childhood through 12th Grade. These plans are underwritten by the United Health Care Student Resources, Dallas, TX, and are serviced by the Hendrix Insurance Agencies, Inc., P.O. Box 119, Lovelock, Nevada 89419.
Supplemental coverage may be provided to students involved in high school, grades 9-12, Interscholastic Tackle Football, by paying an additional premium of $80 to $125.
Extended Dental accidental coverage is available for all the above plans for an additional premium.
Enrollment information is available on-line only. Students who wish to participate in the insurance plans must enroll online at www.k12StudentInsurance.com and go to the Enroll Now link.
Enrollment in the football insurance program must occur before the opening date of football practice – August 13th.

SCHOOL REGISTRATION & ORIENTATION INFORMATION
The Pershing County School District will hold pre-registration for all students who did not attend school in Pershing County during the previous school year.
All new students must provide a birth certificate and immunization record.
High School: All returning high school students or new students to the district who will be in grades 9-12 who did not pre-register during the spring 2012 semester should contact the school the week of August 20th.
Middle School: Seventh grade students register Wednesday, August 22nd from 9:00 a.m. - 12:00 p.m. and 1:00 p.m. – 3:00 p.m. at the middle school. Eighth grade students register Thursday, August 23rd from 9:00 a.m. – 12:00 p.m. and 1:00 – 3:00 p.m. Sixth grade students register Friday, August 24th from 8:00 a.m. – 1:00 p.m. at the middle school.
Elementary School: Students can pre-register beginning, Monday, August 20th from 8:00 - 12:00 and 1:00 - 3:00 p.m. at the Lovelock Elementary School Office. Kindergarten students must have reached 5 years of age on or before September 30, 2012. First grade students must have reached 6 years of age on or before September 30, 2012.
Immunization: New students must also provide written evidence of completion of immunization from either a doctor or the community health nurse on the day of registration. If you have questions, please contact Mrs. Debbie Pontius, school nurse, at 273-1147.
Sports Physical: All students wishing to participate in any extra-curricular activities must get a physical prior to the beginning of the sport season.

The Pershing County School District recognizes its obligation to provide overall program accessibility throughout the School District for disabled persons. Contact the Pershing County School District Coordinator to obtain information regarding the existence and location of services, activities, and facilities that are accessible to and usable by disabled persons.
DIRECTORY INFORMATION:
The Pershing County School District has designated the following information as Directory Information: student's name, address, parent/guardian's name, student's participation in school activities, awards, date of birth, height, and weight for athletic team members, photographs, honors and distinctions, or other personally identifiable information which would not generally be considered harmful or invasion of privacy if disclosed. Directory information may be released to agencies, military recruiters, institutions or businesses for the purpose of providing students with yearbooks, class rings, graduation announcements, athletic apparel, school pictures, scholarship opportunities, or other purposes that benefit the student and/or the school. Directory information will not be released when the purpose is primarily for commercial or sectarian use. Parents who do not want the Pershing County School District to disclose directory information from their child’s education records without prior written consent, must notify the District in writing within two weeks of this publication notice or enrollment in school.
INTERNET USE:
Pershing County School District students have direct Internet access for educational purposes. Parents must notify the Principal's office in writing if they do not wish for their children to participate in this activity. All students new to the district, grades 6-12, are expected to sign the Student Network/Internet Agreement and abide by the policies as stated in the Acceptable Use Policy.
VIDEO SURVEILLANCE PRACTICE:
The schools use video cameras on district property to ensure the health, welfare and safety of all staff, students and visitors to district property, and to safeguard district facilities and equipment. Video cameras may be used in locations as deemed appropriate.
The district shall notify staff and students through this publication that video surveillance may occur on district property. Video recordings may become a part of a student’s educational record or a staff member’s personnel record. The district shall comply with all applicable state and federal laws related to record maintenance and retention. Students or staff in violation of Board policies, administrative regulations, building rules or law shall be subject to appropriate disciplinary action. Others may be referred to law enforcement agencies.
NRS 391.097 TEACHER QUALIFICATIONS – Pursuant to NRS 391.097 this is notification that a parent or guardian may request the professional qualifications of their child’s teacher as to his/her degree(s) and area(s) of certification. Information can be requested from the school district office during regular business hours.
ANNUAL NOTIFICATION OF RIGHTS UNDER FERPA
The Family Educational Rights and Privacy Act (FERPA) affords parents and students over 18 years of age (“eligible students”) certain rights with respect to the student’s educational records. They are:
· The right to inspect and review the student’s education records within 45 days of the day the District receives a written request for access.
Parents of eligible students should submit to the school principal (or appropriate school official) a written request that identifies the record(s) they wish to inspect. The principal will make arrangements for access and notify the parents or eligible student of the time and place where the records may be inspected.
· The right to request the amendment of the student’s education records that the parent or eligible student believes are inaccurate or misleading.
Parents or eligible students may ask the district to amend a record that they believe is inaccurate or misleading. They should write the school principal, clearly identify the part of the record they want changed, and specify why it is inaccurate or misleading.
If the District decides not to amend the record as requested by the parent or eligible student, the District will notify the parent or eligible student of the decision and advise them of their right to a hearing regarding the request for amendment. Additional information regarding the hearing procedures will be provided to the parent or eligible student when notified of the right to a hearing.
· The right to consent to disclosures of personally identifiable information contained in the student’s education records, except to the extent that FERPA authorizes disclosure without consent.
One exception, which permits disclosure without consent, is disclosure to school officials with legitimate educational interest. A school official is a person employed by the District as an administrator, supervisor, instructor, or support staff member (including health or medical staff); a person serving on the Board of Trustees; a person or company with whom the District has contracted to perform a special task (such as an attorney, auditor, medical consultant, or therapist); or a parent or student serving on an official committee, such as a disciplinary committee, or assisting another school official in performing his or her tasks.
A school official has a legitimate educational interest if the official needs to review an education record in order to fulfill his or her professional responsibility.
Upon request, the District discloses education records without consent to officials of another school district in which a student seeks or intends to enroll.
Schools may also disclose, without consent, “directory” type information such as:
· Student Name (Student names are prohibited on the Internet.)
· Major field of study
· Participation in officially recognized activities and sports
· Weight and height of members of athletic teams
· Degrees and awards received
· Photograph
However, schools must tell parents and eligible students about directory information and allow parents and eligible students one week to request in writing that the schools not disclose directory information about them.

The Pershing County School District has prepared the Local Plan for the provision of special education services to disabled youngsters for the 2012-2013 school year. This plan assumes compliance with applicable state and federal regulations and will enable the district to receive federal special education funding.
